- For residential set up, find the application at www.painesville.com/utilities/utilityforms. If you are renting, there is a separate form. The landlord must fill and the tenant must sign. Include a legible copy ID of the person requesting the service. Include a deposit in the form of check or money order: $100 for electric service only or $175 for both water and electric services. Customers can mail the form, copy ID and deposit to Painesville Utilities, P.O. Box 601, or drop this information into one of the Utilities drop boxes located behind the Painesville City Fire Station off of E. Erie Street or on the back steps of City Hall. A. Call our Utilities Billing office at 440-392-5797. There is a fee of $10 to do the test. The fee is refunded if your meter tests bad.

A. You will need a Loosen & Refasten Permit which can be obtained at the Utilities Billing office at 440-392-5797.
