For and in consideration of the CITY OF PAINESVILLE (“CITY”) allowing the undersigned (volunteer name) to perform volunteer clean-up activities throughout the city, (the “Property”), I/we hereby agree as follows:

1) I shall take all precautions necessary for the safety of and prevention of damage to myself and the property of CITY and for the safety of and prevention of injury to other persons when I am performing the voluntary clean-up activity on the Property. I agree that all work shall be performed entirely at my own risk.

2) If the person performing the work is a minor, then I/we agree that at all times the minor will be supervised by an adult and if there is no supervision by an adult, then the minor who is performing the work is prohibited from doing any work of any kind on the Property.

3) I/we agree to indemnify and hold harmless the CITY, their employees and agents from and against all claims of any kind including bodily injury, illness, or death, damages, losses and expenses arising out of the clean-up activity and any activity related to the clean-up activity on the Property. I further waive any claim for injury or damage that I/we may have against the CITY, their employees and agents in the event that the person who is performing the volunteer work is injured in any way or in the event that the person who is performing the volunteer work injures someone else.

Clean Up Day t-shirts are given to volunteers as a thank you for helping Keep Painesville Beautiful! It is also any easy way to identify other volunteers when picking up trash. Register by May 1 for guaranteed t-shirt size! After this date, t-shirts may not be available, however, staff will do their best to match extras.
